Emotions run high at the practice when Dell's orphaned daughter, Betsey, shows up. Amelia and Pete can't agree about a risky surgery for a pregnant woman with chronic pain.

Violet was shocked to see Betsey, Dell's daughter, come out of the elevator at Oceanside. Violet is happy to see her. This quickly faded when Betsey's Aunt Monica tells her that she cannot care for Betsey anymore and leaves. Betsey is left at Oceanside with Violet. Betsey asks Violet where she is going to sleep tonight.
Violet offers Betsey to stay with her for a wile. Betsey accepts her offer. In the morning, Violet freaks out when she sees that Lucas is not in his crib. It turns out that Betsey took him downstairs with her to watch cartoons. Sheldon thinks Betsey needs therapy for all the trauma she has been through. He tells Violet that the best way to help her is to let her go.
In the kitchen at Oceanside, Betsey accidentally spills a drink on the floor. She looks horrified, as though she is going to be in big trouble. To show Betsey that it is ok, Violet pours her own drink on the floor. The two of them take all the sodas in the fridge and shakes them up and makes a mess of the kitchen. Betsey asks to stay with Violet forever. Violet wants to adopt Betsey but Pete is against this idea. He thinks it will be too much for them to handle. Pete asks Cooper to go talk to Violet. Cooper tells her that she should not start try to fix things with Betsey, but she should do that with Luke.
Addison and Pete's patient is a pregnant woman suffering from excruciating pain, but they cannot figure out what it is. The pregnancy is making things worse. She is a few months away from her due date and says that if the pain does not go away, she does not want to have her baby. Amelia discovers that the pain is coming from something putting pressure on her brain. Pete is against the women getting surgery because it could potentially kill herself and the baby. Addison, Amelia, and the woman think surgery is the best option. The surgery turns out to be a success.
Betsey's situation gets everyone thinking. Sam and Addison start thinking about having children. Cooper plays with the idea of Charlotte taking Betsey in. Violet asks if anyone is willing to take Betsey in, but no one speaks up. Violet is devastated and has to go tell Betsey. Violet hands Betsey over to a social worker. She tells everyone that they just did a bad thing.
