Private Practice is a spin-off of the highly popular ABC series, Grey's Anatomy starring Kate Walsh as Dr. Addison Forbes Montgomery, a renowned neonatal surgeon. After realizing that her life isn't advancing at Seattle Grace, Addison packs her bags and heads off to Santa Monica to start a new… More life. She reunites with her newly divorced med school friends, Naomi (Audra McDonald) and Sam (Taye Diggs), and joins them in their chic private practice.

The series, created by Shonda Rhimes, is produced by ABC Television Studio in collaboration with The Mark Gordon Company and Shondaland Productions. It first aired as an episode of Grey's Anatomy. The episode, titled The Other Side Of This Life, was known as a backdoor pilot. It tested the idea of the show, but also shared the plot with Grey's Anatomy scenes weaving in and out, from which it was spin-off.

A comatose, pregnant patient puts a strain on Addison and Sam's relationship when they disagree over treatment. Pete and Charlotte are in an ethical dilemma about whether to reveal an auto accident victim's HIV status. Violet gets caught up in Lucas's first Halloween which puts her at odds with Pete's family.

A woman in a coma is discovered to be pregnant, and Addison and Sam are at odds with each other about what should be done. Addison wishes to get the police involved because this is basically rape, but Sam wants to keep things quiet for now, especially since the woman's husband, Bob, has been very diligent about his loyalty to his wife.

However, it turns out that Bob was the man who had sex with his wife while she was still in a coma. Addison and Sam's fight escalates, and Addison even says that she's withholding sex from Sam until they work through this. Later, complications arise with the woman and she is rushed to surgery, where she miscarries. After Addison hears Bob say that he wants them to keep trying to have kids, Addison calls the police. Sam is very angry, but he says this shouldn't get in the way of their relationship because its a work issue. Tomorrow, he'll still stay furious at Addison, but tonight, he wants to have sex...

Meanwhile Violet wonders why Pete doesn't talk much about his family, specifically his imprisoned mother and his estranged brother Adam. As they take Lucas out trick-or-treating, Pete tells Violet that his mother had a lot of men in her life, but there was just one who'd treated Pete like he cared about him. Pete's mother got drunk and stabbed the man to death. Pete notified the police but Adam lied about the circumstances behind the attack to protect their mother. Pete hasn't forgiven his brother for this.

Cooper and Charlotte are at odds with whether they should try to have kids, and he tries to bribe her with a mini Alabama Lane Cake. Later, Charlotte is finishing up work at the hospital, but then is attacked just as she leaves her office. She is struck in the face and tossed back into the room.
